HRT cost calculator: the first-year number
The cheapest all-in first-year path on the women's wing is Gala Health at about $828, Transparency Grade C. A published $69/month HRT plan with multiple named treatment routes, online care and shipping; exact prescription and checkout terms confirmed during assessment. On the men's wing the cheapest all-in path is MangoRx at about $1,288. Both figures are a year, and that is the point of this tool: the consult fee, the required 12-month plan, the three-month billing block and the membership that buys no medication all disappear from a monthly sticker and all reappear in the annual total.
The calculator will not rank a per-visit fee against a monthly subscription, and it will not add a medication estimate to a provider that bills the drug elsewhere. Those numbers are not published, and a guess would make every group look comparable while making every total wrong. Every assumption it does make is printed below, on screen, before you read a single figure.

Our total therefore runs low for a first-year patient by the difference between the two rates.
- Documented paths override the arithmetic.Where a provider carries a researched year-one figure, that figure is used and its assumption note is printed on the row. Those numbers come from a specific route through the provider's own price list, usually its cheapest systemic-estrogen option.
- No invented medication cost. Providers in the care-only group bill the drug through your pharmacy and publish no price for it. We add nothing, and we label the group instead.
- No lab estimate. Where a provider publishes no self-pay lab price, we print the absence rather than a placeholder.
- Nothing is stored. Your selections stay in this browser tab and are never sent anywhere.

What a monthly price leaves out
Four pricing mechanics live in the gap between the sticker and the year, and all four are documented on this roster. A one-time consult fee is charged before the first monthly charge. An advertised rate can require a 12-month commitment, so the low number and the escape hatch cannot both be true. Prescriptions can bill as three-month supplies, which triples the amount that leaves your account per charge without changing the monthly figure at all. A membership can be a real recurring charge that buys no medication, so the drug is a second bill you have not seen yet.
